Please enter the name of the confirmation candidate so that we can link his/her confirmation name and their sponsor information to their profile.
 
Confirmation Saint Name
Please fill out the information regarding your confirmation name. Please put N/A for any field that is not applicable. 

As a reminder, your Confirmation name should be a saint name. For example, my name is "Erin Elizabeth Prokop" but I could not use "Erin" as my Confirmation Name since there is no canonized St. Erin. I could, however, use "Elizabeth" since there are multiple St. Elizabeths.
 
 
 
 
 
Sponsor Information
Your sponsor must be able to get a sponsor certificate from their home parish. This form will need to be signed by their parish priest and returned to Katie Rosen at the St. Basil parish office. This certificate confirms that they are a registered and participating member of the Christian community.

Reminder: Sponsor cannot be a parent. They must be a confirmed Catholic, in good standing with the Church, over the age of 16.
