I(parent/guardian of above named child) hereby give approval for my child to participate in the Service Project at St. Augustine Hunger Center on Thursday, August 5, 2021 from 8am - 1:30pm. I understand that my child will be driven by a St. Basil adult to the Hunger Center, where he/she will prepare and serve a meal to those in need in Cleveland and do waive, release, save and hold harmless and indemnify St. Basil the Great, Life Teen Core Members, organizers and agents from any personal injury to my child.

By typing my name below, which shall constitute my electronic signature, I acknowledge that I am the parent or legal guardian of the child(ren) named in this form and have the authority to sign this document and act on his/her or their behalf. We will meet for 8am mass at St. Basil the Great and drive to St. Augustine for our 9am shift start. We will return back to St. Basil around 1:30pm.

Masks are not required by the Hunger Center.
